Providers Eligibility

An entity should define in its Policies & Procedures what conditions must be met for a provider to be qualified to write 340B prescriptions and orders (e.g. active medical staff, consulting staff, contracted staff, etc.)  

• The entity should maintain a list of criteria the entity uses to document provider eligibility.   
• The entity should state how often, and by whom, the provider list is updated and disseminated to the pharmacy or pharmacy administrators.
