Patient Eligibility

Patients must have a documented medical service:

• At an eligible and registered location and
• Provided by an eligible provider.

Entities should ensure regular self-audit practices to document:

• The qualifying medical service is on or before the date the prescription is written.
• The patient has a qualifying medical service within a reasonable time increment for ongoing use of 340B drugs.
• Entities should define these criteria in their Policies & Procedures.
